MEMBERSHIP AGREEMENT

1. OVERVIEW

This Membership Agreement outlines the terms governing participation in My-Health membership programmes. By enrolling in a My-Health membership, you acknowledge that you have read, understood and agreed to the terms set forth below.

2. MEMBERSHIP STRUCTURE

My-Health offers structured membership tiers designed to provide preventative and optimisation-focused healthcare services.

Each tier includes a defined scope of services, diagnostics and clinical oversight as outlined on the My-Health website or in your membership proposal.

Membership benefits may vary depending on tier selection.

3. COMMITMENT PERIOD

Paid memberships operate under a minimum commitment period, as specified within each tier.

During the initial commitment term:

  • Membership cannot be cancelled.

  • Fees are non-refundable.

  • Services must be utilised within the active membership period.

The commitment structure ensures continuity of care and measurable health outcomes.

4. FEES & BILLING

Membership fees are billed on the 1st of each month unless otherwise agreed in writing.

Accepted payment methods include:

  • Credit card

  • POS transaction

  • Bank transfer

  • Cash

In the event of a failed payment:

  • Automated retry attempts may occur.

  • Members are responsible for maintaining valid payment details.

 

My-Health does not impose late payment penalties but reserves the right to suspend services in cases of persistent non-payment.

5. UPGRADES

Members may upgrade to a higher tier at any time.

Upgrade pricing will be calculated proportionally based on:

  • The difference between current and upgraded tier pricing.

  • The remaining duration of the existing commitment period.

Downgrades during an active commitment period are not permitted.

6. CLINICAL SERVICES & DISCLAIMERS

All medical services are delivered by practitioners licensed by the Dubai Health Authority (DHA).

My-Health programmes focus on preventative and optimisation-based healthcare.

Services are not a substitute for emergency medical treatment. In case of a medical emergency, members must contact emergency services immediately.

Participation requires:

  • Completion of a health intake questionnaire.

  • Provision of accurate medical information.

  • Execution of additional consent forms where required.

Failure to disclose relevant health information may impact the safety and effectiveness of care.

7. PERSONALISED CARE & ADJUSTMENTS

My-Health programmes are personalised and may be adjusted based on:

  • Clinical findings

  • Diagnostic results

  • Physician recommendations

  • Individual progress

Service scope may evolve to reflect medical necessity and optimisation strategy.

8. THIRD-PARTY PROVIDERS

My-Health programmes are personalised and may be adjusted based on:

  • Clinical findings

  • Diagnostic results

  • Physician recommendations

  • Individual progress

9. MISSED APPOINTMENTS

My-Health collaborates with licensed laboratories, imaging centres and specialist providers.

While My-Health coordinates services, certain elements may be delivered by independent third parties.

My-Health is not liable for delays, errors or operational issues originating from external providers.

10. SUSPENSION & TERMINATION

My-Health reserves the right to suspend or terminate membership in cases including, but not limited to:

  • Persistent payment failure

  • Misconduct

  • Abuse of staff or providers

  • Violation of this Agreement

Written notice will be provided prior to termination where appropriate.

Access to services will cease upon conclusion of the notice period.

11. LIMITATION OF LIABILITY

My-Health provides preventative and advisory healthcare services.

To the extent permitted by UAE law, My-Health shall not be liable for indirect, incidental or consequential damages arising from participation in membership programmes.

Nothing in this Agreement limits liability for gross negligence or misconduct where prohibited by law.

12. GOVERNING LAW

This Agreement is governed by the laws of Dubai, United Arab Emirates.

Any disputes arising in connection with this Agreement shall be resolved in accordance with applicable UAE regulations.

13. AMENDMENTS

My-Health reserves the right to amend this Agreement as required for operational or regulatory purposes.

Members will be notified of significant changes via email or website notice.

 

Continued participation constitutes acceptance of updated terms.

14. ACKNOWLEDGEMENT

By enrolling in a My-Health membership, you confirm that you have reviewed and accepted this Membership Agreement.
